Intake/Social History: Effectively Recording the Intake

Service Coordinators have a challenging position. They are responsible for guiding a family through their EI journey, as well as ensuring that all the rights and procedural safeguards are in place. The intake, however, consists of more than just rights and paperwork. Service Coordinators complete a family assessment and are responsible for sharing that information with both the family and providers. The Intake/Social History Form serves as the record of our intake and is the start
of the child’s Early Intervention record.
In order to effectively communicate that information, they need to know how to appropriately complete that form. In this training, we review each section of this form and how the information given affects the evaluation process and what it tells about the family.
