Using the online video “Foundations of Transition for Young Children,” part of the CONNECT Module series from The Center to Mobilize Early Childhood Knowledge, we will facilitate group discussion around the topic of helping to empower and prepare families for life after Early Intervention, through the utilization of Transition oriented Outcomes within the IFSP process.
